High Risk Work Licences, RTV Training offer are; 

Forklift Truck (LF) and Boom type elevating work platform - WP - Elevated Work Platform over 11m (EWP)

You must not carry out specified class or classes of work unless you hold the high risk work licence for that class.
A high risk work licence is recognised in all states and territories of Australia.

Eligibility for a high risk work licence
You are eligible to obtain a high risk work licence if you:
  are at least 18 years of age
  have done a recognised course of training with an Registered Training Organisation (RTO)
  can use English at a level that enables the safe performance of high risk work
  have the knowledge and ability necessary to safely perform the high risk work
  have been assessed as competent by an accredited assessor.

Once you complete the Forklift course with RTV Training, you can then apply for a high risk work licence at Service Tasmania.

You must submit the following documents at a Service Tasmania outlet:
  a completed Licence to Perform High Risk Application form (PDF, 1.1 MB)
 your current Assessment and Interim Summary Assessment provided to you by an accredited assessor (white copy and needs to be within 60 days of issue date) - which you will receive upon successful completion of the training with RTV Training. 
  evidence of identity documentation.

You will need to pay a fee and have your photograph taken.

Service Tasmania will review the documentation and forward it to WorkSafe for processing. If you do not receive your card within 60 days from when you lodged it with Service Tas, please contact WorkSafe

How to renew your high risk work licence

All high risk work licences must be renewed every five years.
